I'd call them back: "On the off chance it becomes relevant, who is your malpractice carrier?" That might focus their attention on meeting a deadline. Then send a written reminder to their office -- by fax so you have confirmation that they received it before the deadline expired.

I saw "OBJECTIONABLE" on an aviation chart for the first time today. The practice obscures at least as much information as it conveys. I'm curious is the FAA publishes compilation of "objectionable" sites?
