Past written test results

How old?

The testing center/company would likely be the one to provide the results, not the FAA.
 
I failed my PPL written first time around. While it doesn't specifically ask for dates, it has been suggested that it looks better if I put the dates of the failure and retest, along with the passing score on the application.

Yes. Past failures are a big hot button for the airlines these days. Unless you have a pattern of failures, it won't hurt your hiring chances, but if you fail to disclose something, and they find out about it after you've been hired, you'll be sent packing. Same thing with speeding tickets and any other infractions or run ins with the law.
