Reporting Lyme DIsease

Filling out the MedXPress form, under 18x, "Other illness, disability, or surgery. I presume I have to report Lyme Disease and Babesiosis, both over 5 years ago (I don't have exact dates any more, and the treating physician has retired), successfully treated with antibiotics at that time, no lasting effects. Will there be any issues or requests for more information?

Does that mean I need more documentation (as I said the doctor is retired and I'm not sure of the exact dates) or is a simple note on the application that it was successfully treated sufficient?

Good question. If OP cannot get his record, would the FAA require any special testing?
