Say one needs to convert from Commercial pilot(has both ASEL and AMEL) to helicopter license, what are the endorsements needed(and how many) for check ride?
 
This is all spelled out very straightforward in 14 CFR 61.
 
You wouldn't be converting it, you would be adding a class and category rating.
 
Train in helicopter, take knowledge test, take practical test and you will have a Commercial Certificate with Rotorcraft - Helicopter rating. Been there, done that.

What endorsements did you need or did you instructor give you for the check ride?

OK, I stand corrected. I don't recall but, "back in the day", you didn't need an endorsement for the knowledge "written, as on paper" test. Had an endorsement for the practical flight test.

You needed to take a written to add a helicopter to your commercial cert?
