I knew about that one and the controversy about the lowering of the PHX B. Probably just swapped the cities in my head since they’re both CFIT but don’t think the automation was the majority of the problem in PHX.
 
I don't get the misprogramming the wrong identifier. Even on the now ancient 430, when you dial in the ID it gives you the name and location of the facility right there on the page you're dialing it in on.

I was flying from MUCF (Cienfuegos) to MUHA (Havana) and the controller cleared us to TIPOS. What he meant was ITPOS but despite our readback, he didn't catch it. When we put it in the GPS, as I recall, TIPOS was in Saskatchewan... we knew it was wrong and figured it out, but had someone just left it alone, they'd be WAY off course.
