Is that airplane fuel injected? (I assume). The switch of tanks is the obvious one... keeping the mixture leaned, then panicking and not being able to restart? Seems like a long shot...
I mean I know it is polarizing amongst pilots, but it is tough to argue in certain cases that CAPS saves peoples lives no doubt. I am sure if my engine went out, I would be happy to have a chute as a backup.

Yikes. We had a Navajo go down here in Vancouver years ago from Wake turb. Trailing a 767 I believe. Cargo ops, both crew dead. Nothing to take lightly.

TBH, in this one I would give the crew a pass. It sounds like the cockpit door had blown open during the decompression, and that the cockpit in general was a bit of a mess of papers... not to mention the crew probably wanted to get off the airplane/debrief/hit the bar... Whatever... not to point...
