Real question for @lbfjrmd -
My most recent 3rd class resulted in a letter that said I will need CACI for Hypertension at my next medical (~5 months from now).
Can I opt for BasicMed instead of doing the CACI? Is there any difference there?
Also, if I go BasicMed and then need a 2nd class when I "retire" and want to instruct or pick up some other revenue flying will that be looked at unfavorably?

@Ravioli I held a First Class when I was at the airline for years as my HBP was controlled and my physician provided the AME a letter stating it was under control, what medicine I was taking, and BP readings. Shouldn't be a problem when ya retire. As for BasicMed, I dunno as it's too new and I know nuthin' about it really.

I asked this of a Very Senior and Very Special AME (whom we all know and 99% of us love):
"Is it legal to be a CFI with only a basicmed or is the 2nd class required?"
The response from VSaVSAME:
"It is perfectly legal (6,000 lbs and under). Basic med, however only covers PIC. IT does not work for "required crew" (e.g. safety pilot)."
Translation: Instrument training by CFII with pilot who is already PPL or better needs to be clarified. Student pilot under the hood is still a student pilot and the CFI is still the PIC.

if all is well with your BP ... it is seamless for you. If i put 'CACI qualified hypertension' in block #60 you are automatically issued without further FAA interaction. No unfavorable marks ... you can change at will as long as your health permits.
BradZ would be better than one of the doctors on that point.
The literal read is that the basic med safety pilot (whether they are a CFI or not, whether giving instruction or not) must be PIC. It would behoove those involved to make sure they are filling those roles. This is an inane quirk in the reg. The only place I know of when adopting a position of authority (PIC) requires fewer credentials than if you aren't in that position.

Final Approach
So just to clarify, a private pilot (non-CFI) on basic med cannot act as a safety pilot? That's what I was getting from all this. I just renewed my 3rd class anyway but it's a nice thing to know.
